Contact your school		 Upon completion of your online registration, please contact your attendance clerk to schedule an appointment for Kindergarten registration:

The following items are required for the registration appointment:
  • Proof of Residency (credit card bill, deed, DOT identification card, driver’s license, lease, property tax bill, utility bill, tax bill)
  • Birth certificate, hospital certificate, or baptismal certificate (no photocopies)
  • Immunization records
  • IEP, Chapter 15/Section 504, or gifted (Chapter16) paperwork or other Special Education papers, if applicable
  • Complete the following three (3) forms, print them and bring them with you to your registration appointment:
